National Disability Insurance Scheme (NDIS)

The National Disability Insurance Scheme (NDIS) provides financial support for people living with a disability. It’s a whole-of-life approach tailored to each person’s needs and goals.

Supports are provided for people with permanent and significant disability to achieve their goals, participate in their community, and be part of the workforce.

Heart 4 Care presently supports NDIS Participants who are Self Managed.

You may be eligible for the NDIS if you:

  • Are under 65 years of age
  • Have a permanent and significant disability
  • Are an Australian Citizen and/or permanent resident

Assistance with daily activities

Our experienced and caring staff can assist you with a range of daily life tasks such as:

  • Personal care activities – showering, getting dressed, moving around the house and much more
  • Assistance with household tasks – cleaning, laundry, dish washing, preparing meals and more
  • Assistance with daily life tasks – in a group or shared living arrangement

Assistance with travel/transport

If you need a bit of extra assistance to get around, we can provide you with safe and convenient transport to help you get to:

  • School or another educational facility
  • Work
  • Your doctor or other medical appointments
  • Your local shopping centre
  • Family events

Community nursing care

If you need specialist nursing care relating to your disability, our registered nurses will come to your home to provide training to you, your carer, or our care staff to ensure that you have the skills and support you need to live independently at home.

Self Management

As your nominated provider, Heart4care will take responsibility to provide quality care and support services to your loved one’s— but without taking full control like a plan manager would. Heart4care is designed to assist you in handling the admin side while still giving you full control over how your funds are spent.

What supports do the NDIS provide funding for?

Allocated NDIS funding depends on the needs of the individual participant. The NDIS will fund supports that are reasonable and necessary and will assess individuals looking for assistance under the following circumstances:

  • The support offered has to relate to the disability of the participant
  • Funding does not cover costs unrelated to the disability support needs
  • Support must be a good value for money
  • Support can only be given if it will benefit the participant

NDIS supports will also consider the support offered by the participant’s family, home carers, community and their wider network.
